Zero-Calorie Fasting Brews
Whether a drink breaks a fast depends entirely on what you are fasting for, and much of the confusion comes from applying one fast's rules to another's purpose.
Define the fast first
Time-restricted eating for general health or weight management. The relevant question is calories. Plain black coffee, plain tea and water are effectively calorie-free and do not meaningfully affect this.
Fasting before a blood test. Follow the specific instruction given. Many lipid and glucose panels permit water only. Coffee can affect some results, so "black coffee is fine" is not a safe assumption — ask the clinic.
Fasting before surgery or a procedure. Follow anaesthetic instructions exactly. These exist to reduce aspiration risk and are not the same as dietary fasting rules.
Fasting for autophagy. Popular claims here far outrun the human evidence. Most autophagy research is animal or cellular, and thresholds asserted in popular sources are not established in people.
What is genuinely calorie-free
- Water, still or sparkling
- Black coffee — around 2 kcal per cup
- Plain tea of any kind — negligible
- Herbal infusions — negligible
What is not, despite frequent claims
- Milk of any kind, including a splash. Small, but not nothing.
- Bone broth, which contains protein and calories.
- Anything sweetened, including honey.
- Butter or oil in coffee, which is substantially caloric. It may suit a ketogenic approach; it does not suit a calorie-based fast.
Non-nutritive sweeteners are contested. They contribute no meaningful calories; whether they provoke a cephalic-phase insulin response is debated and the evidence is mixed. If your fast is calorie-based, they are unlikely to matter.
Practical cautions
Coffee on an empty stomach provokes symptoms in some people, particularly those prone to reflux. If a fasted coffee leaves you uncomfortable, that is a reason to move it rather than to persist.
Caffeine on an empty stomach is absorbed faster, so the effect can feel sharper. If you feel jittery when fasted at your usual dose, reduce it.
Hydration deserves attention during longer fasts, and electrolytes matter if the fast is extended.
Who should not fast without advice
Anyone pregnant or breastfeeding, with diabetes — particularly on insulin or sulfonylureas — with a history of disordered eating, who is underweight, or who takes medication requiring food. Fasting can affect medication timing and blood glucose in ways that need proper planning.

Is the evidence on zero-calorie fasting brews conclusive?
Rarely. Most beverage research either studies an isolated compound at a dose well above what a cup delivers, or observes populations where the drink is entangled with lifestyle. The honest summary is usually a plausible, partially evidenced mechanism rather than an established clinical effect.
What matters more than which drink I choose?
Serving size, concentration, preparation method, total daily intake and timing. Café portions are frequently several times a nominal cup, and that single factor often explains more variation than beverage type does.
When should I speak to a professional?
